Topic: Reducing Readmissions for Psychiatric Patients through Medication Assessment

The directio of the Project:

Implementing and Evaluating the Change in Practice: The implementation of the change will involve conducting a study on the effectiveness of medication assessment at reducing readmissions among psychiatric patients. For the study, the Project Investigator (PI) will enlist the participation of psychiatric nurses in a survey on the use of medication assessment in psychiatric care. The psychiatric nurses will be given educational material on medication assessment and evaluated on their perspectives on the utility of medication assessment as a strategy for lowering readmissions. The key outcome that will determine the success of the intervention is a significant consensus among psychiatric nurses regarding the viability of using medication assessment in routine practice as a means of enhancing adherence and reducing readmissions.

Intervention instrument: Instrumentation-You must use a reliable and valid instrument, survey, or tool with documentation of the reliability scores in a publication or on a website. A tool should not be created or altered/adapted to avoid error and bias rendering your study useless. Permission for using the instrument, survey, or tool must be placed in the Appendices. This may be in an email or on a website.

The survey instrument that will be used to collect the pertinent data for the SPP is from Giannetta et al. (2020) State the name of the tool. State what the reliability and validity coefficients are

Intervention: Provide a detailed explanation of the educational intervention for the study. Create 3 SMART objectives for your intervention to clarify for the participants. What will be taught to the population of choice and how will it occur? Create a timeline for the intervention as to how long will it take? Should not have an intervention that is longer than necessary and appeals to the audience so that participation is high. Should be based upon national guidelines for a practice change and fills the gap in the literature that you may have found. Some mention of the theoretical framework concepts should be done as well. Be sure to provide an exemplar for the participant or a take away such as a completed simulation, case study, or scenario.

EBP Model: In addition to the model for evidence-based practice (EBP) change proposed by Rosswurm and Larrabee, be sure to outline exactly the step-by-step process on how you’ll obtain site permission, educate nurses about the use of medication assessment tool using educational material/ self-paced educational module, pre/post test, etc.

Data Analysis: Explain how your data will be collected and analyzed. Utilizing IntellectusStatistics will assure that the analysis is correctly presented. Describe the types of statistics you’ll run- primarily you’ll be running a t-test, frequencies (if there’s nurses’ demographic information). You can try to think about what else you may want to collect as well to run analyses on
